I had applied for pan card back in 2009. The agent through whom I did it made some problem in it so as a card?

I had applied for pan card back in 2009. The agent through whom I did it made some problem in it so as a card number is generated but the card has never reached me. When I tried to apply for another pan card in 2011 again the agent said as a number is already there, new card cannot be made, one has to , in such case retrieve the actual card. Now i have been trying for the same for past seven months, I have got a receit of acknowledgment but still the card is not there. To make matters worse I have also lost the receit. and even in the receit that was there, my date of birth was put wrong by the fellow. I need the card as soon as possible and need to make it error free as well. Please suggest what to do in such a situation. Need help.
